Patient Forms/Pre-registration Link
For your convenience, many of our patient forms are now accessible electronically. These forms will be sent to you in a pre-registration link by email or text message 3 days prior to your scheduled appointment and should be completed as soon as possible. If you are unable to complete these required forms electronically please arrive 15-20 minutes prior to your scheduled appointment to complete them in office(NEW PATIENTS should arrive 30 minutes prior). Completing patient forms prior to your visit will enhance the registration process and assist in an efficient check-in on the day of your appointment.
In addition to completing forms electronically for secure upload to your chart, the pre-registration link will also allow you to:
- Upload Insurance Cards/update insurance information
- Update/add Pharmacy
- Update/add demographics such as: address, phone numbers, email address, PCP
- Update/add Emergency Contact/HIPAA
Please have the following items readily available during pre-registration link/In office on the day of your appointment:
- Insurance card (cards)
- Photo identification
- CD(s) of any prior imaging
- List of all current medications, including dosage and frequency
- Pharmacy name, address and telephone number
- Valid Referral (If your insurance requires a referral, please know it is patient responsibility)
- Co-payments/co-insurance/Deductibles must be paid at the time of service
We kindly ask that you arrive 15-20 minutes before your scheduled appointment time to allow us to properly register you for your appointment. All necessary forms and check-in procedures must be completed before you can be seen by a provider. For this reason, completing the available pre-registration link prior to your visit is advantageous.
Patients arriving late for scheduled appointments may be required to reschedule.
